Researchers at the University of Calgary are inviting Alberta physicians to help shape a clinical decision support tool that predicts heart faliure readmission risk and supports more effective discharge planning.
Adults living with heart failure are invited to share their experiences and perspectives on hospitalization, discharge planning, and clinical decision support tools. Your input will help researchers develop a tool that is responsive to the needs of people living with heart failure.
